Formalities after amputation follow an order. Below you will find a practical step-by-step guide from someone who has walked this path.
First a specialist doctor or a specialist physiotherapist issues a prescription for the prosthesis. You do not need a disability certificate for that. With the prescription you go to a prosthetics provider that has a contract with the NFZ, and the provider settles the reimbursement. Apply for the certificate straight away though, because without it you will not get PFRON or PCPR funding, and the decision takes time.
